Terrence McDonagh is a New Orleans Police sergeant, who receives a medal and a promotion to lieutenant for heroism during Hurricane Katrina. Due to his heroic act, McDonagh injures his back and becomes addicted to prescription pain medication. He then finds himself involved with a drug dealer who is suspected of murdering a family of African immigrants.

Two unpopular teenagers, Gary and Wyatt, fail at all attempts to be accepted by their peers. Their desperation to be liked leads them to "create" a woman via their computer. Their living and breathing creation is a gorgeous woman, Lisa, whose purpose is to boost their confidence level by putting them into situations which require Gary and Wyatt to act like men.

On New Year's night, Sandro suffers a heart attack and needs to have an emergency transplant. He ends up receiving the heart of Isadora, a transvestite owner of a beauty salon who had died a few moments before, victim of a runover. Soon, low supplies and a band of merciless smugglers add even greater urgency to their task.
