A unique action-packed drama that tells the story of a Yakuza man who craves blood like crazy, unaware that he has fallen into a trap set by his younger brother.

The R of the title stands for the young protagonist, Rune, fearlessly played by Pilou Asbæk. Imprisoned for violent assault, he's a cocky, good-looking young man placed in the hardcore ward, where his survival depends on quickly learning the prison's parallel world of rules, honor, and obligations. R also stands for Rachid, a young Muslim prisoner who becomes Rune's friend and accomplice, defying the rigid racial stratifications among the inmates.

Fujio Yamadai, the third generation leader of Japan's largest organized crime group, Tamba-gumi, has passed away due to illness. Yamatatsu group leader Tatsuo Yamanaka (Hakuryu), who has become acting group leader, recommends Tamba group leader Kazuhisa Takemoto (Hitoshi Ozawa) to Kashira. However, Sumiko, the sister of the third generation Yamadai, who actually runs the Tanba-gumi, mentions that Takemoto is the man who will inherit the fourth generation of the Tanba-gumi. The story of William da Silva Lima, the last survivor of the group that founded the Comando Vermelho at the end of the 1970s. It shows him living together with the political prisoners incurred under the same National Security Law and his leadership in the Ilha Grande Prison creating a type of unheard of conduct and solidarity in the Brazilian prisons. The film goes on to tell of the group's actions on the streets of Rio de Janeiro during the beginning of the 80s when they irritated the police with their daring robberies, as well as portraying the surprising love story between William and Tereza. The narrative weaves its way between the intimate conflict and the spectacular assaults and escapes.
