196X. Anna, an enigmatic and attractive professional kidnapper, abducts The Professor during his wedding ceremony on request from one of the latter's former girlfriend. This woman, whose face Anna has never seen, wishes to die with The Professor before he marries. Leaving the church with The Professor on the back seat of her motor-bike, Anna starts on the journey to the fictional city of 'Alpha Paris'.

Shibuya, Tokyo, is bustling with the Halloween season. A wedding is being held at Shibuya Hikarie, where Detective Miwako Sato of the Metropolitan Police Department is dressed in a wedding dress. While Conan and the other invited guests are watching, an assailant suddenly bursts in, and Detective Wataru Takagi, who was trying to protect Sato, is injured. Takagi survived and the situation was settled, but in Sato's eyes, the image of the grim reaper that she had seen when Detective Matsuda, the man she had been in love with, had been killed in a series of bombings three years ago, overlapped with Takagi's.

Learning that island of Delos has been taken over by Neuroi, Charlotte E. Yeager and Francesca Lucchini are teamed up with Hanna-Justina Marseille and her partner Raisa Pöttgen to draw out the Neuroi hiding within it. Francesca, who has strong family ties with Delos, objects to their superior's plan to destroy the island, so they work out a new plan to have Hanna and Raisa draw out the Neuroi while Charlotte and Francesca drop bombs on it. When this plan ends up failing, Francesca tries to attack on but is inevitably forced to retreat. The next day, before the bombardment on the island is due to take place, Charlotte, unwilling to let Francesca down, decides to make use of an abandoned ship's winch to pull the Neuroi out of its hiding place, allowing it to be destroyed without any damage to the island. On this man-made island in Tokyo Bay, the bonds between twins are pushed to the limit in the Ultimate Athletic Tournament! The tournament consists of three insanely difficult obstacle courses. Teamwork and human drama are forged within these grueling challenges. Participants are limited to twins, but exactly what constitutes a "twin" is unclear, resulting in some unusual pairings. In the past, controversial appearances have included clones, adopted siblings, or triplets in shifts.
