Arthur Conan Doyle reveals the story behind Sherlock Holmes and his mysteries by telling about Dr. Joseph Bell, from whom he drew his inspiration, after meeting him as a medical student in Edinburgh. This TV movie served as the pilot for the later released minisseries Murder Rooms: Mysteries of the Real Sherlock Holmes produced by the BBC. The series then picks up with Sir Arthur Conan Doyle's time as a general practitioner in Southsea, solving mysteries with the help of his mentor, Dr Joseph Bell, who is still based in Edinburgh.

At the request of Inspector Lestrade, Sherlock Holmes investigates a double murder on a theater stage. Suspect number one is Irene Adler. In the investigation, as always, he is assisted by Dr. Watson.

Dr. Watson, who served in the English armed forces and was in the Afghan war, retires and returns to his homeland, in England. Since the financial situation of the doctor is very precarious, his long-time friend Mr. Stamford offers him to rent a room in the house at 221-B Baker Street, which is rented by an elderly lady - Mrs. Hudson. The second rented room is already occupied by another gentleman - the mysterious Mr. Sherlock Holmes. Holmes makes an ambiguous impression on Watson. He conducts complex chemical experiments with blood, plays the violin, has the deepest knowledge about cigar ash, London dirt and criminal law, but at the same time demonstrates complete ignorance of well-known truths (for example, the fact that the Earth revolves around the Sun), does not read fiction, as well as books on history and philosophy. At the same time, very strange visitors constantly come to Holmes, and on the table he has portraits of personalities of a disgusting appearance.

Impersonating Little Eva in a third-rate travelling production of Uncle Tom's Cabin fails to earn Nora enough for a square daily meal, and to make thing worse, her stage career comes to a sudden end when the sheriff arrives with a writ of seizure. Nora hops a passing freight but is frightened by a tramp, jumps off, and literally rolls into the town of Wattelville. After being arrested as a suspicious character, Nora is adopted by the kindly Ma Forbes, whose son James works in the local bank. "Pug" Hennessy and "Soup" McCool, two crooks, inveigle the scrupulously honest James into a poker game and, as a result, he is forced to steal $300 from the bank to cover his losses. Having learned his lesson, James proposes to Nora, who never again is forced to go hungry.

After being captured by T-Man Jack Blaisdell and sentenced to 20 years of hard labor on a counterfeiting rap, Tiger McGuire is sprung from jail by his old gang. Vowing to get even with Jack, Tiger kidnaps the young man's fiancée, Helen (Holmes), bringing the girl to his hideout, a fancy yacht owned by Lucy Carlisle.

Jimmy Rudge, chief of the infamous Gang "Jacks of Spades", kidnaps the Marquis d'Harcier and passes himself off as him. The impostor held large parties at his castle, in order to rob the invited guests. Charley Colms, a quirky private detective, gets hired to unmask the Marquis as a fake.
